Completing the I-693
- Book your first visit by either emailing or calling us. Send us your vaccination records via email beforehand.
- The first visit can be done either over the phone or in-person. We will review your records and let you know which vaccinations still need to be completed. We will send any laboratory test orders to you.
- The next visit will occur in the office. You will be examined by Dr. Jonathan Yeh. If all requirements are met, you will be given the completed paperwork. If not all requirements have been met, such as vaccinations or lab results, then a follow-up appointment may be required.

Form I-693
The patient portion must be completed in black ink without any errors. Download the latest version of the I-693 form here. Download the instructions for completing the I-693 form here.

Government-Issued ID
We accept government-issued IDs that include a picture of yourself, such as passports and driver's licenses. Bring a marriage certificate if your current last name does not match your government-issued ID.

Immunization Records
Please bring any available records, including COVID vaccination cards. Non-English documents should be translated by a certified translator. Having these ready may reduce the cost of your visit.

How long will the process take?
Assuming that your lab results are normal, we will usually have the paperwork ready within one week after your first appointment.
How much does the exam cost?
The immigration process requires different labs and vaccines depending on age. So, the price of the exam will depend on your age. The average exam cost ranges from $370 to $490. Vaccines are an additional cost. Please contact us if you have further questions regarding the cost.
Will I get a copy of the completed I-693 form?
Yes. We will email you a copy of the form when you come to pick it up.
Do you accept insurance?
Unfortunately, we cannot accept insurance for the immigration exam.
Can I have the required vaccines done by my primary care physician (PCP) or at a pharmacy?
Yes, it is completely fine to have the required vaccines done by your PCP or at a pharmacy. We will let you know which vaccines need to be administered after we receive your lab results. After the vaccination, you will need to send us the vaccination record via email.
Do I need a translator?
Dr. Jonathan Yeh speaks English, Spanish, Portuguese, and Mandarin. If you do not speak any of these languages, then you should bring a translator.
