About the exam

• An appointment is required. We do not accept walk-ins at this time.
• The appointment usually takes less than 30 minutes.
• We recommend bringing some items to your DOT physical exam. Alternatively, you can email us the materials prior to your visit:
• Eyeglasses or hearing aids, if needed
• List of current medications
• Physician-related documentation: the last 3 progress notes, if available, will be needed from your primary care doctor
• Records of any current treatment related to sleep apnea, cardiac, or orthopedic device use
